How to Establish Care
For New Patients
Welcome! We are so glad you found us. If you are considering establishing care with Green Tree Pediatrics we think that there are a few things that are important to know about us.
What we ask of GTP Families:
In order to make the care experience at GTP special, we invest our resources in the people and the technology that will ensure the best clinical care experience we can provide. To make this possible, we ask a few things of our families.
Live within a 45 minute drive to either office
Follow the recommended schedule of Well Child care
Use the Patient Portal
Maintain a credit card on file
Know the ways we make it possible to reach medical advice from the practice 24/7
Establish Care with 3 steps:

Check below to ensure that GTP participates with your insurance and is accepting new patients
Click on the button below to schedule a time to complete digital registration paperwork. This is a virtual link which can be completed from the comfort of your own home. After you schedule, you will receive a link to check in up to 24 hours before the scheduled time. Please allow 15-20 minutes to complete the process. You will complete all the digital paperwork needed to establish care. You will need to complete the appointment prior to the scheduled time.
Once you complete the Registration process, someone from the GTP team will reach out to you with next steps!

For Current Patients
Patients who do not keep their appointments deprive others of an opportunity to see their doctor. Please call us as soon as you know that you will not be able to keep an appointment, so that we can offer that time to another patient who needs it. You will be charged for missed appointments. Missed well care appointment fees and missed sick visit appointment fees are $50. We will contact you if you are more than 15 minutes late so that we can reschedule your appointment.
Missed Appointments & Late Arrivals
Green Tree Pediatrics is a pro-vaccine practice and we believe in vaccines as one of the pillars of preventative medicine.
We follow the American Academy of Pediatrics schedule for vaccines as a standard of care. We encourage all parents to vaccinate their children. We also follow the AAP guidelines and respect a parent’s right to refuse treatment on behalf of their children.
We have put together a recommended vaccination schedule as a guideline for the health and well-being of your child.
During our well-child visits, we’ll review upcoming vaccinations together and provide any information you need on protecting your child from dangerous, preventable diseases.
If you decline vaccines or follow an alternate schedule, please know that we will discuss our recommendations at each well visit and we will expect parents to sign a waiver at each visit listing the declined vaccines.

Since you are in an HMO, your child will be covered for services received from Plan providers and for emergency care.
Some HMOs offer benefits that are tailored specifically to your situation and others have reciprocal agreements with plans in other areas. Check with your plan.

It is your responsibility to notify our office if there are insurance changes. If you fail to share insurance information that results in a claim rejection, you will be responsible. It is your responsibility to know your individual coverage.

Self-pay patients are required to make an advance payment on their office visit charge.

If you have a deductible, coinsurance, or non-covered service, you will receive a bill in the mail. Payment within 30 days is appreciated. After 60 days, your account will be assessed a $25 fee for non-payment. After 90 days, we reserve the right to postpone treatment and also to dismiss your child from the practice for non-payment.
We accept cash, personal checks, Visa, MasterCard, American Express, Discover, Debit Cards and Health Savings Account Cards. If your bill is sent to collections, you will be responsible for all charges incurred in this process. If there are financial hardships, we are happy to discuss payment plans.

You can provide credit card information for use at a later date, to pay the medical bill. The card on file will be used for copays, deductibles, non-covered services paid out of pocket, or for portions of bills not covered after insurance has paid out its portion.

We realize that it may take some time for newborns to be added to an insurance policy. However, it is very important that you provide the insurance company with the information that they request in a timely manner. Failure to do so may result in delays of coverage and greater financial costs to you.

Additional Information
Health Appraisals and Sports Physical Forms are often needed for pre-school and school age patients. It is best to have these filled out at the time of their annual well visit. Otherwise contact the office to have these completed.

Should you require a copy of your child’s medical record, you may obtain it from the patient portal at no charge to you.
Medical Records
It is the patient’s responsibility to notify the office in a timely manner when refills are necessary. Approval of your refill may take up to three (3) business days, so do not wait to call. If you use a mail order pharmacy, please contact us fourteen (14) days before your medication is due to run out.
Medication refills will only be addressed during regular office hours (Monday – Friday (8:00am – 5:00pm). Please notify your provider on the next business day if you find yourself out of medication after hours. No prescriptions will be refilled on Saturday, Sunday or Holidays.

At Green Tree Pediatrics, we take pride in the coordination of your care. Depending on your insurance company, some require patients to obtain a referral from their primary care provider in order to see a specialist. Our Referrals Coordinator will carefully assess your physician’s orders and will be more than happy to help you find the specialist needed for your child. If needed, please call our office to schedule an appointment with the doctor to assess your child and obtain a referral, please call us at 734-475-4500.

If your child is already a Green Tree patient, you can schedule an appointment online. You’re also welcome to call us at (734) 475-4500 to speak to the receptionist.
If you are new to Green Tree, you can call to schedule an appointment to be seen in the office or schedule a free ‘Meet the Doctor’ appointment to learn more about us.

We make sure to keep about half of our appointment slots open every day for children who need to be seen.
If your child gets sick during our office hours, please call us at (734) 475-4500 or schedule an appointment online.

We care for children from birth to 21 years. For well child care, we follow the schedule recommended by the American Academy of Pediatrics. You can view our Well Child Guide above

Children do not get sick on schedule, which means that you will sometimes need medical advice or treatment outside our regular office hours.
If it is a medical emergency that requires immediate attention, call 911.
For after hour advice there is always a provider available to help.
Options for families
Symptom Checker -- is a powerful tool with good advice for families after hours - this is a tool that is endorsed by Healthychildren.org website--that is an educational site from the American Academy of pediatrics. This has a Red - Yellow - Green system of flagging complaints based on age of whether you should seek care immediately(red), the next day(yellow) or self care(green).
After hours phone advice--We have a Nurse Triage available for our patients. If any questions that require a provider's input we are at the ready 24/7 to assist you.
We offer urgent care hours on Saturday mornings. If you think your child will need to be seen, you can call the office between 8 am and 10 am to schedule an appointment for Saturday morning. Clinic hours will be held in the Chelsea office.

It’s easy to switch to Green Tree Pediatrics. Simply call us to establish care and we can help you from there. You are welcome to first schedule a free ‘Meet the Doctor’ appointment to meet us and take a tour of the office.
If you are transferring care from another office, we can help you with the paperwork to transfer your child’s medical record.

If your child is a minor (less than 18 years), we need authorization from a parent to be able to see them. If you need to send a child aged 15-17 years old by themselves, please fill out this consent form and either fax it or have the child bring it with them to the office.
We are unable to see a child younger than 15 years if they are unaccompanied by an adult.

Yes. Vaccines are safe and necessary to ensure the health of both your child and the people around your child. In the U.S., we have made great progress in eradicating contagious and even deadly diseases using vaccines.

We are committed to ensuring the safety of your child. We take many precautionary steps to ensure a safe clinical environment for your child.
Throughout the day, we are wiping down surfaces and cleaning the office. We thoroughly clean exam rooms after each visit.
When there is a concern for contagion, we ask parents to call from their car, so that we can bring their child straight to an exam room bypassing the reception area.

Before you deliver, you should be comfortable with your pediatrician!
If you already have children who are patients at Green Tree Pediatrics, contact the office to notify us of your due date and the hospital at which you plan to deliver.

Sometimes, it can be difficult to bring up health issues with children. It’s important to make sure they know that they can come to you with problems so they feel comfortable letting you know of any potentially embarrassing symptoms.
If you are concerned that something is going on and your child will not discuss it, bring them to the doctor for a check-up so we can assess your child’s health needs.
